O\u0160TE\u0106ENJA MOZGA IZAZVANA RECIRKULACIJOM"},"content":{"rendered":"<p><a href=\"\/index.php?page_id=1408\"><img decoding=\"async\" src=\"\/wp-content\/uploads\/2023\/02\/3.png\" alt=\"\"><\/a><\/p>\n<h1>\n<h1 class=\"western\" lang=\"bs-BA\">ETIOPATOGENEZA KAROTIDNE BOLESTI<\/h1>\n<\/h1>\n<div>\n<h3>\n<h2>3.4 O\u0160TE\u0106ENJA MOZGA<br \/> IZAZVANA RECIRKULACIJOM<H2><\/h3>\n<\/div>\n<div>\n<p class=\"western\" align=\"JUSTIFY\">Tokom ishemije mozga metabolizam glikoze je poreme\u0107en i anaerobnom glikolizom se nagomilava laktat; \u010dinioci transportnog sistema poprimaju karakteristike slobodnih radikala; osloba\u0111anje dopamina je povi\u0161eno; poja\u010dana je razgradnja fosfolipida i nagomilava se arahidonska kiselina i javlja se ekscitotoksi\u010dnost.<\/p>\n<p class=\"western\" align=\"JUSTIFY\">Reperfuzija tkiva u kome su se odigrali svi ovi ne\u017eeljeni doga\u0111aji ne dovodi do oporavka, ve\u0107 do progresije o\u0161te\u0107enja. Tako nastaje tzv. reperfuziona povreda ishemi\u010dnog mozga <i><b>(reperfussion<\/b><\/i><i><b> <\/b><\/i><i><b>injury<\/b><\/i>). Periferno tkivo oko centralnog, ishemi\u010dnog ognji\u0161ta biva dalje destruirano. Reoksigenacija i normoglikemija u novo pristigloj krvi deluju kao hiperoksemija i hiperglikemija. Zbog nemogu\u0107nosti da iskoristi glikozu (metaboli\u010dki mehanizmi su paralisani) javlja se hipermetabolizam (poja\u010dana glikoliza) koji vodi pove\u0107anom stvaranju laktata. Nastala acidoza (laktocidoza) dovodi do novog o\u0161te\u0107enja tkiva. Drugi razlog<\/p>\n<p class=\"western\" align=\"JUSTIFY\">lezija tokom reperfuzije je prekomerno stvaranje kiseoni\u010dkih slobodnih radikala u kontaktu ishemi\u010dnog tkiva sa molekularnim kiseonikom. Slobodni kiseoni\u010dki radikali lediraju vulnerabilne \u0107elije. Razvija se lipidna peroksidacija plazma membrana, i nastaje citodestrukcija.<\/p>\n<p class=\"western\">Slobodni kiseoni\u010dki radikali se tokom reperfuzije najvi\u0161e stvaraju:<\/p>\n<ul>\n<li>\n<p class=\"western\">u procesu oksidativne fosforilacije, jer enzimi ne mogu da metaboli\u0161u ponu\u0111eni molekularni kiseonik;<\/p>\n<\/li>\n<li>\n<p class=\"western\">u procesima sinteze prostaglandina i leukotrijena;<\/p>\n<\/li>\n<li>\n<p class=\"western\">degradacijom oslobo\u0111enog dopamina;<\/p>\n<\/li>\n<li>\n<p class=\"western\">sintezom azot-oksida degradacijom adenozina, odnosno hipoksantina koji podle\u017ee dejstvu ksantin-oksidaze.<\/p>\n<\/li>\n<\/ul>\n<p class=\"western\" align=\"JUSTIFY\">Revaskularizacija ishemi\u010dkog mozga (hirur\u0161ka, interventna, tromboliti\u010dka) stvara dodatne probleme. Svaka arterijska rekonstrukcija zahteva izvesno vreme klemovanjailiblokadearterijskecirkulacije.Timeseishemi\u010dnotkivododatnoli\u0161ava krvotoka. Recirkulacija omogu\u0107ava pojavu hipermetabolizma sa posledi\u010dnom laktoacidozom i sintezu slobodnih kiseoni\u010dnih radikala. Zato recirkulacija mora biti pra\u0107ena preparatima koji stabilizuju postishemi\u010dni metabolizam i spre\u010davaju nastanak slobodnih kiseoni\u010dnih radikala, uz antiedematoznu terapiju. Da bi se spre\u010dilo pretvaranje ksantin dehidrogenaze u ksantin-oksidazu treba davati lekove: \u201c\u010dista\u010de\u201d slobodnih radikala, i blokatore kalcijumovih kanala.<\/p>\n<p class=\"western\" align=\"JUSTIFY\">Hiperglikemi\u010dna sredstva u terapiji akutnog ishemi\u010dnog mo\u017edanog udara su kontraindikovana. Budu\u0107i da i normoglikemija mo\u017ee u ovoj situaciji delovati kao hiperglikemija.<\/p>\n<\/div>\n<p><!--more--><br \/>\n<!--
